Survey

Primary Care

We have a short 5-minute survey about primary healthcare (like GPs, pharmacies, and community clinics). Would you like to take part?
Do you or your whānau live in a remote, isolated, or small rural area?
Do you have a regular GP or medical clinic you usually visit?
When needed, can you or your whānau get a same-day appointment for urgent issues (like sudden illness or injury)?
Are you able to book planned appointments (e.g., check-ups, repeat prescriptions, routine screenings) when needed?
Has cost ever prevented you from seeing a doctor or getting care?
Have you or your whānau ever used an Urgent Care Clinic because your GP wasn't available?
Have you had to visit the Hospital Emergency Department because you couldn’t get a GP appointment?
Were you or your whānau given any of the following while at the hospital?
Have you used any online health services that weren’t through your own GP?
Have you or someone in your whānau called a telehealth service for medical advice?
If yes, did the service give you the help or info you needed?
